How do I get my medical records from a Delhi hospital?
Hospitals in Delhi usually keep the official file. You can ask for copies of your own records. This is an admin process, not a PatientSaathi download. We do not store hospital files on this site.
What people usually ask for
- Discharge summary (after an admission)
- Lab, pathology, and radiology reports
- Prescriptions and OPD notes you were given
- Itemised bill (often needed for insurance)
- Indoor case papers, if you need a fuller copy than the summary
Ask the Medical Records Department or the patient-relations / billing desk. Take photo ID, the UHID or IPD/OPD number from your file, and the admission or visit dates.
Put the request in writing
Delhi Medical Council has pointed hospitals and registered doctors to Indian Medical Council ethics rule 1.3.2: if a patient or authorised attendant asks for medical records, the request should be acknowledged and the documents issued within 72 hours. That is a professional-conduct rule, not a PatientSaathi service promise.
Practical steps that usually help:
- Write a dated request listing each document by name.
- Address it to Medical Records and the Medical Superintendent.
- Keep a photocopy or photo of the request with the hospital’s stamp or acknowledgement.
- If they offer only a verbal recap, ask again for copies (paper or PDF).
Government hospitals in Delhi may also accept an RTI request for certified copies. Private hospitals are not covered by RTI in the same way; the written request, then the hospital’s own grievance path, is the usual first step.
PatientSaathi cannot pull the file for you and cannot file the request.

Copies of Delhi hospital records are issued by that hospital’s Medical Records desk. There is no national site that downloads every hospital file. ABHA / ABDM at abdm.gov.in is NHA’s rail — creating an ABHA number does not give PatientSaathi the file.
